This group includes men facing housing instability, oscillating between temporary accommodations, shelters, and the streets. Many are employed, sexually active, and grappling with transient living situations rather than chronic homelessness. HIV street medicine is emerging as a pivotal solution to bridge this gap. This innovative care model brings testing, prevention, and treatment directly to individuals in their environments, eliminating the need for appointments or identification. By shifting the burden of access onto the healthcare system, street medicine makes prevention a tangible reality through immediate education, access to medications, and consistent follow-up.

Evidence from existing street medicine programs shows promising results. Teams operating in urban areas with high housing instability have successfully engaged men who previously avoided care, initiating treatment and maintaining ongoing support. The immediacy of same-day testing and treatment initiation significantly reduces the risk of losing contact with patients.

Despite the positive outcomes, street medicine is not yet mainstream. Structural barriers persist, as most healthcare funding remains linked to facility-based billing models, which do not accommodate care delivered in unconventional settings. This creates a challenge for street-based initiatives to secure necessary resources.

Moreover, workforce limitations hinder the expansion of street medicine. There is a lack of training programs for clinicians focused on the unique skills needed for this approach, including building trust in non-institutional environments. Until funding, training, and policy frameworks evolve to support this model, street medicine will remain a niche rather than a standard practice.

Ultimately, HIV street medicine addresses critical gaps in care delivery. Rapid testing and immediate treatment initiation are key components that prevent delays in care. By adapting healthcare to align with the realities of patients' lives, this approach fosters greater engagement and improves health outcomes. Ending the HIV epidemic hinges not on new medical breakthroughs but on effective delivery systems that reach those most in need.
