Experimental Drug Targets Fat Cells for Destruction, Not Just Shrinkage

Experimental drug CBL-514 triggers fat-cell self-destruction; early trial findings suggest it may reduce abdominal and visceral fat, though larger human studies are still needed.

Experimental Drug Targets Fat Cells for Destruction, Not Just Shrinkage

Most weight-loss approaches reduce the size of fat cells, but those cells generally remain in the body. The experimental drug CBL-514 takes a different route: it is designed to trigger their orderly self-destruction.

Developed by Taiwanese biotechnology company Caliway, CBL-514 blocks the protein DYRK1B, activating apoptosis, the body's regulated process for clearing cells. The drug is injected directly into targeted areas of fat rather than taken to suppress appetite.

Early human findings

In a Phase 2b study involving 108 adults with abdominal fat deposits, MRI measurements indicated a 20.3% average reduction in subcutaneous abdominal fat 12 weeks after treatment began. Participants received up to four courses of injections or a placebo.

Researchers also analyzed visceral fat measurements from two Phase 2 trials, presented at the European Association for the Study of Diabetes meeting in 2026. Among 23 participants who received CBL-514, visceral fat volume was 12.01% lower than baseline four weeks after the final treatment. At a later follow-up, it remained 10.45% below baseline. The 17-person placebo group showed increases at both time points.

Animal experiments have also raised the possibility that removing some fat cells could affect weight regain after other treatments stop. However, those results have not been demonstrated in people, and remaining or newly formed fat cells may still expand.

CBL-514 remains experimental and is not an approved obesity treatment. Larger studies will be needed to establish its safety, effectiveness and lasting impact. If confirmed, this cell-targeting strategy could broaden future options for managing body fat.
